During their approach, prior to a troop extraction, the Huey (UH-1C, tail number: 66-00714) banked left, struck the ground and the RPM deteriorated. The aircraft struck three dikes and inverted. Two of the four man crew were killed in the aircraft accident. The crew was:

At the time of his death, Specialist Wood was survived by his mother, Jackie A and his father, Harold Shelby Wood, Sr, Lewisburg, Kentucky. His father, a Veteran of World War II and Korea as a Chief Warrant Officer, passed away 10 years later, and both are buried in Rose Hill Cemetery, Central City, Kentucky.
