At 1807 hours, 18 October 1968, a Dust Off helicopeter, UH-1H, tail number 66-16760 was shot down by enemy fire. It had 7 individuals on board with two scout dogs. The aircraft and burst into flames and three passengers, one dog, and a crew member managed to get off the aircraft. The crew member died en route to medical facilities.

Reported Missing in Action on 18 August, were WO Cole (recovered/declared dead 22 October); WO Doolittle (recovered/declared dead 25 October); SGT Hernandez (recovered/declared dead on 21 October); and SP4 Dieffenbach died en-route to medical facility. The other 3 passengers survived.

The men who perished as a result were from the 54th Medical Detachment, 498th Medical Company (Air Ambulance), 67th Med Group, 44th Med Brigade, US Army Vietnam:
