He graduated from C.K. McClatchy High School in Sacramento, California.

The Crew Chief, SP4 R.F. Buehner heard a humming noise and then the aircraft lost power. After the autorotation, it crashed, nose low, and rolled over one time. An observer noted that the aircraft had low rotor RPM on impact. The crash claimed the life of WO1 Terrance W. Nelson. The three other crew members were injured.

Later analysis revealed a short shaft failure which was not detected by the tech inspector, a maintenance discrepancy.

WO Nelson was placed on tbe Seriously Injured list on 13 May and died 23 May 1967. He was survived by his mother Irene Jo Nelson (1914-2004) and father James Royce Nelson (1918-1986), a WWII veteran, SSGT US Army Air Corps. They are all buried in Odd Fellows Lawn Cemetery and Mausoleum, Sacramento, California

In addition to the medals above, he was awarded a Republic of Vietnam Military Merit Medal and as all American soldiers, he would be awarded the Vietnam Cross of Gallantry (Unit).
