A Note from The Virtual Wall

On 10 Jan 1968 a UH-1B (tail number 64-14011) from the 128th Assault Helicopter Company went down after being hit by small arms fire. The gunner, SP4 Stephen M. Childs, was able to get the other crewmen out of the wreckage and call for assistance using an emergency radio. The three survivors (WO Hartwell had been killed in the incident) were picked up by another helo, but that aircraft too was hit on lift-off, killing 64-14011's crew chief. The two dead were CWO William R. Hartwell and SGT John C. Tingley of Kathryn, North Dakota.
