SSG George La Fayette Coody was killed when a flare went off inside his Armored Personnel Carrier. Another soldier, PFC Christopher Williams was burned on over 52% of his body and survived 21 days after the incident; he died at Brooks General Hospital Burn Unit, Fort Sam Houston, Texas.

Because it was a non-combat incident, it may be that the reason SSG Coody was awarded a Soldiers Medal for saving Williams or other soldiers on or inside the vehicle. SSG Coody died from smoke inhalation.
