Additional information about this casualty:
Mid-air with B/158 Lancer aircraft flown by CWO Richard C. Worthington, WO1 Robert L. Kirk, SP4 Frank Hernandez, SP4 William C. Weiss. The crew chief of Poe's ship was the only survivor after falling through triple canopy jungle, by jumping or being thrown out upon collision. Both aircraft were smoke ships. They collided while flying in formation laying a smoke screen for a combat assault in the mountains west of Tun Tavern and Langley. Lancer was leading, with Poe echelon right when they collided. Poe was short with one or two weeks left on his tour. Against the advice of his friends he chose to fly. He took smoke ship, feeling it was safe duty
